In order to integrate javascript or php form into wordpress CMS there are several steps required:

  • Copy and paste the form code of your into blank file (form.php) and upload the file to the root or the theme directory of your hosting. This is needed because when putting the code directly into the wordpress editor, some of the charecters and symbols are interpreted differently and functionality of the form is lost.
  • Create a Wordpress page template for the form page and include the form.php file with the form code on the body of the created template to load the content of the form.php file created in the step 1.
  • Finally, add a new page from the admin end of the worpress CMS and apply created page template.
